By entering my name above, I hereby grant Nevada Donor Network Inc. (NDN) the use of my name and likeness for the purpose of promotion of organ and tissue donation by NDN. This grant of permission includes a non-exclusive, irrevocable, royalty-free, worldwide license to any copyright and other intellectual property rights inherent in the use of my name and likeness by NDN for the above-stated purpose. I understand that the use of my name and likeness may include, but not be limited to, photographs, audiovisual works, newspaper articles, brochures, displays, television, radio, websites, promotional videos, social media or any other community services material. I hereby acknowledge that this authorization is granted without obligation of any kind on the part of NDN, its directors, officers, employees, and designated agents. This authorization is given without hope or expectation of reward or compensation of any kind. I hereby waive my right to inspect or approve any materials that may be created by NDN that may include my name, image, photo, likeness or voice. I, together with my heirs, assigns, agent(s), guardians and legal representatives hereby release NDN, its directors, officers, employees, and designated agents, from any and all claims, liabilities and losses that may arise from the use of my name, image, photo, likeness and voice. I hereby state that I have all right, title, interest, and authority necessary to give this authorization to NDN and grant the rights to NDN set forth above.
